Designating Memorial Gifts to Hospice

There are many ways you can help Hospice continue to provide its essential services to others in our community. Designated Memorial Gifts to Hospice offers a meaningful expression of one's sympathy, love and respect while remembering family members and friends.

Whent he time comes, you may designate the Hospice Foundation as beneficiary of memorials sot hat family and friends can make donations to Hospice in memory of your loved one. The folowing language may also be added to a memorial notice in support of Hospice:

Appropriate acknowledgements of memorial gifts are always sent to both the family and donor.

The opportunity exists for lasting recognition of your loved one. When cumulative memorial gifts total $1,500 or more in an individual's name, that person's name will be inscribed on a plaque in our yearly Memory Garden.

How to Make a Tribute or Memorial Gift to Hospice

Remembering people keeps their spirits alive in our hearts. You can remember someone with your donation to Hospice. If you let us know the name and address of someone to notify of your gift, we will send a card letting them know of you thoughtfulness. Gifts of $50 or more will be recognized in our quarterly newsletter, DATELINE.

Hospice does not disclose the amount of the gift, just the donor's name.

Your gift to the Hospice Foundation supports care for the terminally ill and their loved ones, regardless of their financial circumstances.

Mailing your Gift

1. Make check payable to: Hospice Foundation
2. Include a note indicating who the gift is in memor/honor of
3. Please include the name and address of the family member you wish us to notify of your donation (the amount is never mentioned).
4. Mail to: The Hospice Foundation of WNY, 225 Como Park Boulevard, Cheektowaga, NY 14227-1480

We will promptly:

1. Acknowledge your gift and send you a receipt
2. Notify the person(s) whom you requested that we notify of your thoughtful gift.
