Hospice Buffalo offers emotional, spiritual, social and medical care to patients who are nearing the end of their lives. Each patient has an interdisciplinary team that includes a physician, nurse, social worker, grief support counselor and volunteer. |

The Hospice Buffalo House offers emotional, spiritual, social and medical care to patients who are nearing the end of their lives. Each patient has an interdisciplinary team that includes a physician, nurse, social worker, grief support counselor and volunteer. |

Home Care Buffalo provides skilled nursing and other health care services patients who require in-home support for all types of injuries and illnesses. Our special programs for heart and lung patients provide an interdisciplinary team approach toward the management of Congestive Heart Failure and Chronic Obstructive Pulmonary Disease. |

Essential Care is a special way of caring for a child with a life-threatening illness. Our program, the first of its kind in the United States, is a pediatric program of The Center for Hospice & Palliative Care. Our program works closely with The Children's Hospital of Buffalo and Roswell Park Cancer Institute. We give a child and family complete and consistent care - physical, spiritual and emotional - at home or in the hospital. |

The Life Transitions Center offers a variety of services to help people cope with death related issues. Programs are available for adults, children, families and individuals. It is the Center's goal to ensure that no one who is grieving or living with a serious illness will have to struggle alone. |

The Center for Excellence in End-of-Life Education, Research & Practice provides training and educational activities designed to teach palliative care at the undergraduate, graduate and postgraduate levels. The Center for Excellence also offers educational programs for the lay community on issues related to caregiving, self-advocacy and advance directives. |

The Hospice Foundation of Western New York accepts, administers and invests all charitable funds on behalf of The Center for Hospice & Palliative Care. All gifts are tax deductible as provided by law. The Hospice Foundation was established to support the work of The Center for Hospice & Palliative Care and to solicit funding for future needs. |

Caring Hearts Home Care is a licensed home health care service agency that provides a plan of care specifically designed for our Hospice Buffalo and Home Care Buffalo patients. |

CMI Education Institute provides education and training for health care professionals, counseling professionals and the general public on topics related to palliative care, bereavement, serious illness, grief, loss and/or other subjects relevant to the field of thanatology. CMI’s membership organization, the American Academy of Bereavement, offers conferences and seminars across the United States and is devoted to the education, preparation and advancement of bereavement specialists. |
